The determination of vitamin B12 is of limited specificity and sensitivity for the measurement of the vitamin B12 status, especially with concentrations below 300 pg/ml. Signs of a clinical vitamin B12 deficiency can also be found in patients with vitamin B12 concentrations in the lower reference range (< 300 pg/ml). In these patients with levels of 150 - 300 pg/ml, holotranscobalamin should be determined additionally. For an optimum homocysteine level, the vitamin B12 level should be above 340 pg/ml. |
