Therapeutic range: Min. level: 10 - 15 µg/ml Max. / peak level: 20 - 40 µg/ml For life-threatening infections and pathogens with reduced sensitivity: Minimal level 15 - 20 µg / ml. Elimination half-life: 4 - 10h (adults), 2 - 3h (children), 6 - 10h (newborn). A decreased renal function can cause an elevation of the Vancomycin concentration into the toxic range. Patients with decreased liver function show a delayed elimination of Vancomycin. Vancomycin concentrations above the therapeutic range are ototoxic and nephrotoxic. The ototoxicity is increased if aminoglycosides or furosemide are given simultaneously. |
