Based on our long-year experience with porphyria diagnostics, we have decided to modify our porphyria diagnosis protocol to a stepwise protocol. This new protocol consists of a first step with HPLC differentiation of the porphyrins which is followed, only in the case of clinically significantly results, by a subsequent second step with the determination of the porphyrine precursors delta aminolevulinic acid and porphobilinogen.

Our decision is based on our long-term observations that there was never a case with a normal porphyrine excretion but clinically significantly increased porphyrine precursors.

Preanalytics/Notes:

Spot urine is only suitable for acute diagnostics! Send in 10 ml of 24h urine sample during a pain-free interval (please state the amount collected), protect sample from light and store in a cool place.

Additional analysis of porphobilinogen and total porpyrines in 24h urine are recommended.
