D-Dimer (Citrated plasma, frozen (cp))

Specimens:	1 ml Citrated plasma, frozen (cp)
Preanalytics/Notes:	Please send in citrated blood immediately. If same-day analysis is not possible, the blood sample must be centrifuged immediately (at least 15 min. at 1500 x g), the plasma removed with a pipette and frozen. No blood sampling from central venous catheter.

Indication/Significance:	To rule out a deep leg vein thrombosis or a pulmonary embolism Diagnosis (staging) and follow up of a consumptive coagulopathy/disseminated intravascular coagulation activation (DIC) The determination of D-dimers to diagnose a deep leg vein thrombosis or pulmonary embolism is not helpful in the following cases and therefore NOT indicated: Anticoagulative therapy for at least 24 h Fibrinolytic therapy for at least 7 days Trauma or surgical intervention in the past 4 weeks Disseminated malignomas Known aortic aneurysm Sepsis, severe infections, pneumonia, severe skin infections, Liver cirrhosis, pregnancy e.g. only with own reference ranges Estimation of the recurrence risk
Interpretation:	The determination of more… The determination of D-dimers serves the diagnosis of exclusion in the case of suspected deep leg vein thrombosis or pulmonary embolism. The test is very sensitive but relatively unspecific, as elevated D-dimer levels can also be found with consumptive coagulopathy, malignomas, fibrinolytic therapy, intra- and post-surgical, stress, haemolysis, sepsis, pregnancy, liver cirrhosis, traumas and aortic dissection. If lysis therapy of the deep vein thrombosis is successful, the D-dimer concentration should increase to the 2- to 3-fold of the baseline value in the first two days. Assessment: a) D-dimer within the normal range: Thromboembolism unlikely b) D-dimer low or significantly elevated: Thromboembolism cannot be ruled out (Cave: unspecific D-dimer elevations!) c) D-dimer elevated in the case of intravascular coagulation activation (DIC) close
