| Testcode: | E_C1ESTP |
| Specimens: | 1 ml Citrated plasma (cp) |
| Preanalytics/Notes: | Withdraw 5 ml of citrated blood, immediately centrifuge blood after sampling, transfer supernatant citrated plasma into a new tube and label accordingly. Note: Citrate plasma is suitable for both analyses (determination of activity and protein). If both of these tests are required, please send in frozen citrate plasma. |

Increased in the case of more… Increased in the case of acute phase reactions. Decreased with hereditary angioedema. C1 esterase inhibitor (C1-INH) is the only inhibitor of complement C1r and C1s and thus the most important regulator of the classical pathway of complement activation. A disturbance of the inhibition leads to the so-called angioedema (Quincke's oedema, glottic oedema). In the case of the congenital form, there can be a deficiency (low C1 INH concentration and activity) or a functional loss (only C1 INH activity reduced). An acquired deficiency (e.g. with B-cell lymphomas) is described as well. With a clinically manifest disease, the activities are usually < 25 % of the standard value. The complement factors C2 and C4 are considerably reduced owing to increased consumption, whereas C3 is normal. C1 INH is increased with acute inflammations. |
